Traditional microneedling and radiofrequency (RF) microneedling both stimulate your skin's own collagen, but they aren't interchangeable. Standard microneedling is gentler, more customizable, and pairs beautifully with add-ons like PRP, while RF microneedling delivers deeper, more dramatic results in fewer sessions.

In this blog, our Aesthetic Nurse Shannon Dellinger draws on her experience performing both treatments at Mehta Plastic Surgery in Palo Alto to help you decide which one fits your skin goals.

What Is Traditional Microneedling?

Traditional microneedling uses an automated sterile applicator with 14 micro points to create uniform micro-traumas in the skin. As the skin heals, it becomes smoother, tighter, and more even in tone. Shannon customizes each treatment by choosing high-end skincare products as a lubricant, since the micro-injuries allow deeper product penetration.

Microneedling at Mehta Plastic Surgery can improve:

  • Acne scars
  • Enlarged pores
  • Fine lines
  • Uneven skin texture
  • Overall skin tone

Patients can also add their own platelet-rich plasma (PRP) to the treatment, sometimes called a "vampire facial." PRP is drawn from your own blood, then spun in a centrifuge to isolate growth factors and proteins that further stimulate collagen and speed healing.

What Is RF Microneedling?

RF microneedling combines the same micro-needling process with radiofrequency energy delivered through the needle punctures. This dual action makes it a more intensive treatment than standard microneedling, which means fewer overall sessions and faster, longer-lasting results.

The ultra-fine needles create microscopic channels in the skin, and RF energy travels through those channels to heat the deeper tissue layers. This triggers a stronger production of elastin and collagen than needling alone. RF microneedling is especially effective for:

  • Age spots
  • Acne scars
  • Large pores
  • Wrinkles
  • Stretch marks
  • Crepey skin

How Do the Two Treatments Compare?

The biggest differences between microneedling and RF microneedling come down to intensity, downtime, and how many sessions you'll need. Traditional microneedling is a lighter touch: most patients need a series of three to six sessions, spaced several weeks apart, with results appearing three to seven days after each visit. RF microneedling goes deeper into the skin, so most patients need just three initial sessions followed by one yearly touch-up to maintain results.

Recovery also looks different. After traditional microneedling, you may notice pink or red skin for a few hours to a couple of days, along with mild flaking. RF microneedling tends to cause mild redness for a few days, though most patients return to their normal routine the next day. According to the American Academy of Dermatology, microneedling (also called collagen induction therapy) works by triggering the skin's natural wound-healing response to rebuild collagen, which is the mechanism behind both versions of this treatment.

Which Treatment Is Right for You?

Your ideal choice depends on how dramatic a result you want and how much downtime you can tolerate. If you're looking for subtle, ongoing skin maintenance with a treatment you can easily customize with PRP, traditional microneedling is likely your better fit. If you want to more aggressively target deeper concerns like crepey skin, stretch marks, or significant textural damage, RF microneedling's added energy will get you there faster with fewer visits.

Neither treatment is necessarily "better" across the board. Instead, think of them as tools suited to different goals. Many of our Palo Alto patients also combine microneedling with other skin rejuvenation treatments like Ultherapy, Sculptra, or BOTOX for a more comprehensive transformation.
